Community Health Services

The Community Health Services Department promotes the health and well-being of Larimer County residents through a variety of health care services including clinics, home visits, health screenings and educational programs. Programs are organized to deliver services in the areas of maternal and child health, reproductive health, adult health, and nutrition. Services emphasize prevention, education, health risk reduction, and early detection of diseases or potentially handicapping conditions.

Program Information

Immunizations
To protect Larimer County residents from vaccine-preventable diseases, and prevent their spread from person-to-person.

Family Planning Services
To make every child a wanted and planned addition to the family by providing reproductive health care, family planning, medical and health education services, primarily to low-income individuals.

Maternity Services
To achieve optimal outcomes for pregnant women in Larimer County by assuring access to early and appropriate prenatal services, promoting healthy behavior and risk reduction during pregnancy, and facilitating postpartum access to family planning services.

Adult Health Services
To provide health screening, assessment, counseling and referrals to Larimer County residents 18 years of age or older relating to cardiovascular disease, diabetes, cancer, and recommended vaccinations.

General Nursing Services
To provide on a walk-in basis a variety of personal health care services which are otherwise available through regular clinics

HCP (Health Care Program for Children with Special Needs)
To provide all children with special health care needs the opportunity to obtain services that encourage and assist their optimal development.

WIC (Special Supplemental Nutrition Program for Women, Infants, and Children)
To provide nutritious foods and nutrition education to pregnant, postpartum, and breastfeeding women and their infants and children under age 5 who have a nutritional and financial need.

Breastfeeding Resources

EPSDT (Medicaid outreach program for Children and Pregnant Women)
To enable Medicaid recipients who are pregnant or under age 21 to obtain periodic physical examinations and other health care benefits in order to prevent, detect, or treat health care problems early.

Health Education and Data
To promote, encourage, and contribute to the health of all people in Larimer County through education, advocacy, and community organization.

Nurse-Family Partnership

  1. To improve the outcomes of pregnancy by helping women improve their health behaviors, and identify emerging obstetrical problems and access to treatment of those problems before they become more serious.
  2. Improve children's health and development by increasing parenting skills.
  3. Increasing economic self-sufficiency of participating families.

Tobacco Education and Prevention
To reduce the burden of disease and suffering of the people in Larimer County caused by tobacco use by reducing initiation of tobacco use by youth, promoting cessation of tobacco use among youth and adults, reducing exposure to environmental smoke, and eliminating the disparities in tobacco use among specific populations.
