Letter of Authorization
- Letter must be on company letterhead
- Date (within last 30 days)
- Dealer Number or Lienholder ID number (if applicable)
- State what the agent is authorized to do on behalf of the company
- Name of the Authorized Agent(s)
- Signature of company representative (must be someone other than the authorized individual)
- A business owner with sole ownership may complete a DR 2444 Statement of Fact form in lieu of providing a letter of authorization
- Original, faxed or photocopy is acceptable
